Bring them with you when you come in for your first visit. If you choose to fill out the patient form packet at our office, please arrive 20-30 minutes before your appointment to allow enough time to get them completed before your appointment.

- Lower Extremity – use for problems associated with any pain or dysfunction from the hip down.
- Upper Extremity – use for problems associated with any pain or dysfunction from the shoulder down.
- Back – any problems associated with low back pain, disc problems back pain with pain radiating into the leg.
- Vertigo/Dizziness – any problems associated with vertigo or dizziness.
- Neck – any problems associated with neck pain or neck pain with radiating pain into the arm.
- Neurological – use for any problems associated with a neurological problem such as a stroke, Parkinson’s disease or multiple sclerosis.
- Balance & Gait – use for problems associated with walking, balance and/or a fear of falling.
